Transition Coordinator/Intervention Specialist Springboro High School
SUMMARY: Under general supervision, provides daily instruction to assigned students; prepares daily lesson plans; supervises participation in curricular, co-curricular, and related activities.
Reports to the building principal.
DUTIES AND PERFORMANCE EXPECTATIONS:
1.
Communicate with students, parents, staff, community/adult service providers, and agencies about issues related to the successful transition of students with disabilities.
2.
Serve as a resource to families in accessing community support and transition services, as well as providing information about transition topics.
3.
Assist students with disabilities to determine appropriate career pathways for successful transition after high school.
4.
Consult with intervention specialists to develop transition goals for students that are necessary for a successful transition from school to adult community/post-secondary opportunities.
5.
Coordinate with intervention specialists to create Behavior Intervention Plans to ensure appropriate behavior for success at the work site.
6.
Attend IEP meetings to discuss transition services and student transition needs.
7.
Administer transition assessments or identify appropriate transition/career assessments for students with disabilities and work closely with intervention specialists in administering these assessments.
8.
Teach or coordinate the teaching of daily living skills and employability skills to students with disabilities.
9.
Develop and coordinate a formal work program for students with disabilities at Springboro High School, including networking in the community to build employment placements for students.
10.
Develop and coordinate job placements in the school, district, and community.
11.
Schedule job placements for students with disabilities and coordinate any needed transportation to job sites.
12.
Facilitate and organize job coaching.
13.
Collaborate and coordinate with community agencies and organizations that support students with disabilities (e.
g.
, Opportunities for Ohioans with Disabilities (OOD), Warren County Developmental Disabilities, Warren County Career Center, etc.
).
14.
Build and maintain a working collection of post-secondary transition opportunities within the community, and provide information to build capacity grades 7-12 in regards to post-secondary options.
15.
Stay informed regarding updates in policies and procedures related to transition at the state, county, and agency level.
16.
Provide in-service training to school staff on transition topics, transition IEP goals and requirements, and new changes or updates in policies and procedures.
17.
Collaborate with transition coordinators in other school districts/counties to discuss and network regarding updates on transition and innovative programs currently being used.
18.
Other duties as assigned by the supervisor.
QUALIFICATIONS:
? Licensed Intervention Specialist (Mild-Moderate or Moderate-Intense)
? TTW Endorsement is not required but preferred
? Experience in writing Transition Plans
? Relevant experience in supporting the vocational needs of students
? Must pass criminal background check.
? Hours: 7.
25 hours per day/ 185 day contract
